Key Responsibilities:

Apply your expert knowledge and experience of relevant legislation, social work theory, policy, and principles to provide exceptional strengths-based practice.
Undertake complex assessments of need, risks, and strengths, and translate them into effective intervention plans.
Advocate for person-centred care, putting people’s views and wishes at the heart of your decision-making.
Form strong professional relationships with partners in a multi-disciplinary context.
Communicate effectively both in writing and verbally, demonstrating excellent written and verbal communication skills.
Recognize and respond to behavior that may indicate resistance to change, ambivalent cooperation, and the need for immediate action.
Keep your knowledge and skills up to date, adhering to Adult Social Care practice standards in line with the national Professional Capability Framework.

Job Roles

We currently have opportunities for passionate and committed Social Workers in various teams across Cornwall, including:

ASC Locality Teams: Mid, East & West Cornwall
Community Hospitals / Discharge to Access: Mid Cornwall
RCHT Hospital Team: Mid/East Cornwall

Qualifications and Experience:

Registered Social Worker with Social Work England.
Expert knowledge and experience of relevant legislation, social work theory, policy, and principles.
Previous experience in a social care environment with a track record of excellent strengths-based practice.
Proficiency in conducting complex assessments and delivering difficult messages.
Strong written and verbal communication skills.
Ability to build strong professional relationships within a multi-disciplinary setting.
